For Patients
Proxy access allows you to securely communicate with a patient’s physician’s office on behalf of a family member or person under your care. This communication is done using a MyChart Account. Proxy access is granted once an authorization form has been completed and processed by a site where the patient is receiving care.
Pediatric Proxy
Pediatric proxy access for MyChart allows a parent or guardian to securely communicate on behalf of an IU Health Arnett pediatric patient who is under the age of 14 and will automatically terminate on your child’s fourteenth birthday due to privacy concerns.
Adult Proxy
Adult proxy access for MyChart allows you to securely communicate on behalf of an IU Health Arnett patient who is 18 years of age or older. In order to obtain access, both the patient and proxy must complete and sign an authorization form.
