Indiana University Health Arnett Internal Medicine providers, Riley Physicians Pediatrics and Family Medicine providers team up to provide services at the IU Health Arnett Medical Offices located at 2600 Greenbush Street, Lafayette. Primary healthcare for patients of all ages is available.
Services offered include treatment for medical illnesses in adults such as diabetes, hypertension and high blood cholesterol as well as men’s health screenings and women’s health screenings. The Riley Physicians Pediatrics team offers services including asthma management, well child and same-day sick appointments, sports physicals and treatment of developmental, behavioral and learning disorders.
Internal medicine appointments are available Monday through Friday from 8 am to 5 pm. Early morning and evening hours are available by appointment.
Family medicine appointments are available Monday through Friday from 8 am to 5 pm. Evening and Saturday morning hours are available by appointment.

Dr. Hart has been practicing with IU Health Arnett since 1993. Dr. Hart and his wife have two children. They enjoy many outdoor activities. He is an avid sports fan, no matter the sport, and enjoys coaching his son’s little league baseball team. He also enjoys listening to many different types of music.
Dr. Schendel, his wife, who is a nurse, and their three sons have enjoyed living in Lafayette since 1993. He feels privileged to be able to treat patients in the outpatient and inpatient settings in this wonderful community.
Dr. Schleck has been in his practice in Lafayette since his completion of his residency in training in 1983. He enjoys the long term relationships with his many patients and strives to give them personal and individualized care while guiding them in the coordination of their healthcare needs.
Jasmine K. Sidhu, DO has been in practice since 2013. She pursued a career in Primary Care because it allows her to personally connect with her patients and treat each individual based on their unique needs. Dr. Sidhu’s practice philosophy is to practice honestly, thoroughly and personally. She believes patients play an important role their healthcare; she strives to educate and empower her patients. In her free time, Dr. Sidhu enjoys bicycling, swimming and spending quality time with her family.
Rose Mae Tantoco, MD chose internal medicine so she could focus on adult medicine yet also have the opportunity to specialize. Her clinical interests include diabetes, obesity and pulmonary care for issues such as asthma and COPD. Dr. Tantoco believes that the physician and patient must work together to create an individualized plan with a goal of health maintenance. In her spare time, Dr. Tantoco enjoys reading mystery novels and gardening.
Throughout Dr. Gibbs’s medical career, her determination and passion for medicine has not wavered. Attending medical school in Detroit and her residency in Chicago gave her a chance to see severe medical illness in a population in need. She chose Internal Medicine because it requires empathy and provides an intellectual challenge. Her training in geriatrics showed her the important role medicine plays in the transition from adulthood into geriatric years and gave her the tools to help the aging patient have the highest quality of life possible. She and her husband are enjoying getting to know Lafayette and know it will be the perfect place to call home.
Colette Colburn, AGPCNP-BC began her nursing career in 2011. Her interest in working with adult and geriatrics populations pushed her to earn her advanced degrees. Colette’s goal is to help patients live their best life possible through a caring and knowledgeable approach to the education and management of their healthcare needs. Colette has been a part of the IU Health Arnett family since 2012. She enjoys being outdoors and being active in her free time.
Abby Merrell, AGPCNP-BC appreciates working with seniors because she can learn so much from her patients; they have a wealth of knowledge and wisdom to share. In her free time, Merrell enjoys spending time with her family, church activities and attending Major League Baseball games.
Lara K. Weeks, MD loves caring for people of all ages and enjoys the continuity of care that she can provide by taking care of families. She believes that preventive care is crucial to the health and well-being of our community. Dr. Weeks focuses her practice on prevention and helping families to develop healthy habits. In her free time, she enjoys spending time with her children and attending their numerous musical and athletic events, running, travel and she is actively involved with her church.
Cindy Fritz, FNP-BC has been practicing as a nurse practitioner for more than 10 years. Fritz enjoys treating all age groups from newborn through the end of life as every age group has its own special challenges. Fritz completed the Certified Medical Examiner examination so she could provide quality care physicals to patients in need of Department of Transportation (DOT) physicals. This allows those patients the convenience of having all of their medical needs handled in one place. Her philosophy is to serve her patients to the best of her ability by treating their mind, body and soul. Fritz also believes in allowing her patients to participate in their quality healthcare. Fritz enjoys reading, traveling and spending time with her family.
Melinda Hendrix, FNP-C enjoys getting to know the entire family and providing care for the patient throughout all ages. She started working as a nurse at IU Health Arnett in 2011 before becoming a nurse practitioner. During her free time, Hendrix enjoys spending time with her family, camping and enjoying the outdoors.
Marshall J. Criswell, MD finds reward in working with children and their families to ensure future developmental, physical and mental health. Originally from Delphi, Indiana, Dr. Criswell graduated from Purdue and completed medical school at Indiana University School of Medicine. In his free time, Dr. Criswell enjoys spending time with family, traveling and watching his favorite teams: the Cubs, Colts and Boilermakers. He and his wife, Alicia, are excited to call Lafayette home.
Dr. Ho is in a unique position to be both a pediatrician to individual patients and the Medical Director for all patients, working everyday with the Indiana University Health Arnett medical care teams to strive for the highest medical quality and best customer service. He gets to provide for one patient at a time and on a larger scale, for all patients, their families and friends, and the community. Dr. Ho says he has the best job in the world; he gets to build a trusting relationship with kids, be a partner to their parents and help lead the IU Health Arnett organization toward one simple goal -- To be the best place for you and your family to be taken care of and feel taken care of.
Dr. Lipp has practiced Pediatrics in the Lafayette community since 1978, providing specialized care in General Pediatrics as well as Developmental and Behavioral Pediatrics. He has enjoyed having many of his patients grow up and return with their own children. Dr. Lipp and his wife, Brenda, raised three children in this wonderful community. He is an avid fan of the Indianapolis Colts and Purdue University football and basketball. His hobbies include photography, hiking and gardening.
Dr. Park practiced medicine in Illinois and Minnesota before returning to her hometown of West Lafayette, Indiana. Her mission is to give patient-centered care that takes into account the unique needs of each child and family. Dr. Park strives to be a compassionate and effective communicator while providing the highest quality care. She believes in a team approach to healthcare, from the front desk staff to every nurse, lab technician and provider involved in your care. Dr. Park is married with one son. She enjoys hiking and spending time with her family.
Dr. Woodman has been practicing Pediatrics since 1999 and joined Indiana University Health Arnett in 2004. She is eager to listen to her patients’ concerns and provide them with comprehensive, family-centered care. Dr. Woodman lives in Lafayette with her husband and two children. She enjoys the Lafayette area, spending time with her family, reading and traveling.
Mary Lou has worked as an RN for more than 10 years, many of those at Riley Hospital for Children at Indiana University Health in the intensive care unit. She has been a Certified Pediatric Nurse Practitioner since 1992. She enjoys Purdue football and basketball and traveling to the beach and spending time with friends and family.
Dr. Ansorge believes the most important aspect of medicine is having a patient-provider relationship that is based on mutual trust and open, effective communication. He chose to practice family medicine because he enjoys ongoing relationships with his patients and the variety of the medical problems that he encounters. He is especially interested in treating children, patients with diabetes and those that require minor procedures. Dr. Ansorge is married and has three children. He enjoys World War II history, cosmology, literature, ice hockey, soccer and University of Michigan sports.
Leslie Bentinganan, DO is a native of New England, but has enjoyed living in small midwestern towns for more than 25 years. She has lived locally for more than five years raising two children. As a past Air-Force medical officer and working with practitioners to enhance the quality of life for her disabled child, she brings a variety of experiences to her practice of medicine and patient care.
Dr. Blumling enjoys treating adult and pediatric patients. He stresses preventive care, including exercise and a healthy diet. Dr. Blumling and his family have lived in Lafayette for more than 20 years
Dr. Hendrick strives to create relationships for his patients, in which people listen, respect and care about each other. He is married and has three children. His family enjoys hiking, swimming in the creek, playing freeze tag, riding bikes and playing “tickle monster.” Dr. Hendrick also enjoys fishing, reading philosophical books and writing nonfiction. He also loves University of Oklahoma football.
Silpa C. Nannapaneni, MD has been practicing in her specialty, Family Medicine, since 2013. Dr. Nannapaneni enjoys the long-term, patient-physician relationship that is fostered in the primary care environment. She enjoys caring for patients of all ages and appreciates the opportunity to address a wide scope of medical issues with her patients. In her free time, she enjoys yoga, running and badminton.
Dr. Whitesides has been providing care as a family physician to families in Lafayette for more than 20 years. He enjoys getting to know his patients, sharing their lives, watching them grow and, most of all, helping them be healthy.
