Why pre-register?
Pre-register for your service, test, procedure or surgery to expedite the registration process in order to decrease patient wait time. We know your time is valuable!
How is a patient pre-registered?
Pre-registration can be completed by telephone the day prior to your outpatient test.
Call the Access Services Department at 765.751.5075 or 1.866.477.3734.
Be prepared to provide the following:
- Date of test and procedure
- Demographic information
- Insurance information (on card)
- Complete physician name(s)
Hours for Pre-registration
The Access Services Office hours of operation are from Monday – Thursday, 8 am – 8 pm, and Friday, 8 am – 6 pm.
