At Indiana University Health Ball Memorial Hospital, patients and families can experience many benefits from receiving skilled services in the home, short- or long-term, as appropriate to the patient’s condition. Our goal is to help patients return to their regular routines and activity levels, whether it be returning to work or school, or living independently or with minimal assistance.
The IU Health Ball Memorial Home Care clinical team is trained to provide care for all ages, from newborns to senior citizens. IU Health Ball Memorial Home Care staff includes nurses, physical therapists, occupational therapists, speech and language pathologists, medical social workers, home health aides, dietitians and pharmacists. Our team members are ready to assist patient and families with transitions from a variety of care settings back into the home environment.
We connect with patients through direct referrals from IU Health Ball Memorial Home Care Managers, physicians, patients and families. We collaborate with your physician to determine the treatments you need to manage your specific condition.
When possible, our IU Health Ball Memorial Home Care Discharge Coordinator will meet with you in the hospital to explain the home care services you will receive. We have a nurse on call 24 hours a day including holidays and weekends. We provide physical therapy on weekends for patients discharged with an orthopedic condition that requires therapy to begin right away.
Our home care clinical services include comprehensive management of diseases and conditions including diabetes, Chronic Obstructive Pulmonary Disease (COPD), stroke, wound care, medicine management and other chronic conditions. When insurance criteria are met, we can also provide home lab draws. We also offer a home care Heart Failure Program, including home telehealth. When needed, we provide equipment, such as breast pumps and sleep apnea aids. Our home care also offers a home medical alert button for free for 60 days.

If you are coping with heart failure, it is important to monitor your vital signs on a daily basis to detect any developing problems early. The IU Health Ball Memorial Home Care Heart Failure Program is designed to keep you out of the hospital and empower you to lead an active life.
After you are discharged from the hospital, we visit your home the first three days and either call or visit for the first seven days to teach you how to manage your symptoms.
We use a telemonitoring system to track your blood pressure, heart rate, oxygen saturation and weight. The telemonitor allows you to talk with a home health provider when you are having symptoms. All of your data is shared with your physician on a regular basis. If needed, an immediate visit can be made to your home, or an ambulance can be called.
If your condition requires home infusion therapy, our home care nurses work with you to provide the fluids and medicines you need and teach you and your family how to use the infusion equipment.
Services include but are not limited to fluids for hydration and injectable antibiotics. When possible, we teach you about your home infusion procedure at your bedside, before you are discharged from the hospital. This helps to reduce stress and ensure the success of home infusion treatments.
If the IU Health Home Care Pharmacy is filling your prescription, your medicine or fluids will be delivered directly to your hospital room or home and a home care nurse will provide nursing services to teach the administration of the treatment.
