A new health insurance option for uninsured Hoosiers

In Monroe County alone, more than 15 percent of our population is uninsured. And, industry experts only expect costs to continue to increase. By 2010, just a little over two years from now, premiums are anticipated to be nearly $1,500 more for families in order to cover and help subsidize the uninsured.

In order to provide more affordable health insurance to those in need, the State of Indiana and Governor Mitch Daniels have developed the Healthy Indiana Plan (HIP), a new insurance option for uninsured Hoosiers.

HIP will provide a $1,100 Personal Wellness Responsibility (POWER) account to each adult enrolled in the plan. A basic commercial benefit plan will cover medical expenses that exceed the amount provided by the POWER account. In addition, each participant will receive preventive health services.
Who is eligible?

To be eligible for the Healthy Indiana Plan, you must meet the following requirements:

  • Age 19 – 64
  • Annual household income up to 200 percent of the Federal Poverty Level (please see the contribution charts on pages 8 and 9 of the FAQ’s for more details)
  • Not eligible for Hoosier Healthwise or Medicaid
  • Not pregnant (pregnancy covered by Hoosier Healthwise)
  • Ineligible for health insurance through your employer
  • Uninsured for at least six months

How do I apply?

Step 1: Determine your eligibility for the plan.

Step 2: Get an application from Bloomington Hospital by calling 812.353.5300 or download and print an application from hip.in.gov.

Step 3: Complete all sections of the application truthfully and as completely as possible in blue or black ink.

Step 4: Sign the application where indicated on page 4.

Step 5: Gather copies of the materials listed on page one of the Information to Get You Started handout (included with application). These materials must accompany your application.

Step 6: Submit the application to the Healthy Indiana Plan. You can submit the application in any of the following ways:

Mail your application and materials from Step 4 to Indiana University Health Bloomington Hospital, Patient Financial Services - HIP, P.O. Box 1149, Bloomington, IN 47402.

Fax your application and materials from Step 4 to IU Health Bloomington Hospital at 812.353.9834.

After December 17, 2007, you may also send your application directly to the State either by mail or fax:

Step 7: Cooperate with the Healthy Indiana Plan on requests for further information or interviews. HIP will contact you by phone or U.S. Mail if additional information or documentation is needed for your application. HIP requests that you respond quickly to any requests for additional information so your application can continue through the process.
