Open Enrollment for changed Life Insurance products: March 3 - 21
As shared during Open Enrollment for 2014, your IU Health Bloomington benefits are being altered to create efficiencies in system-wide Human Resources systems and more consistency across the IU Health system. Related forms are below.
If you’re eligible, you’ve received a letter about changes to your life and disability insurance benefits. You must make any changes/declarations between March 3 and 21, 2014. These changes take effect April 1, 2014 and will first appear on the April 24, 2014 paycheck.
Please take a moment to read the letter and follow any instructions. You must work 48 hours per pay to be eligible for these benefits. Special circumstances may apply to new employees or those who are newly eligible, see letter and forms for more details. Due to the confidential nature, the forms required for this change are available online via the Intranet Home Page, available via any hospital network connected computer. UNUM representatives will also hold educational sessions to help you in the transition.

|For most eligible employees, your Basic Life Insurance benefit is transitioning to one-and-a-half your annual base salary ($150,000 maximum). This benefit is provided to you as part of your employment.||

Supplemental/Dependent Life, AD&D
New options are available for these voluntary benefits.

Short Term Disability
Short Term Disability (STD) is transitioning to a voluntary benefit, paid for by you, the employee. If you elect to purchase this insurance product, it will pay you 50-60% of your base salary after a 14 or 30 day waiting period. Please note that your PTO or IPB hours may also be utilized to cover the difference between the STD insurance benefit and your regular pay if you were working. Those currently receiving short term disability benefits during this transition will continue to be covered under the current policy.

Long Term Disability
|Long Term Disability (LTD) is provided to you as part of your employment. These benefits will be available at 181 days of disability and beyond at 50% of standard wages ($8,333/month maximum). You may also purchase additional coverage at your expense, which is an extra 10% on top of the hospital-provided 50%. Additional coverage will be paid at 60% ($10,000/month maximum) total.||No action required. You will automatically receive the new benefit. You may choose to purchase additional LTD insurance through UNUM. Please see the related forms available online.
