Even if you should be in a situation unable to make medical decisions for yourself, you retain your right to decide what medical treatments you receive and who may speak on your behalf. When you write down these decisions on paper, it is called an advance directive.

Our Pastoral Services department can assist you in establishing your own advance directives, or address any questions or concerns that you may have about them.

Print out and read our Advance Directives Information Packet. The packet includes the following forms:

  • Living Will Declaration
  • Appointment of Healthcare Representative

Remember, it is important for you to convey your wishes about these issues with your doctor and loved ones. Be sure to establish your own advance directives.
