Fragmented care supplied by multiple providers can slow the recovery process. At the Back to Work Center, your employee receives care from one occupational healthcare system throughout the course of treatment. Seamless care also provides both employers and insurance carriers one source for complete treatment information.

Nurse Case Manager:

The key to successful treatment within specialty care is an effective nurse case manager. The Back to Work Center’s Nurse Case Manager is Pam Hoppe, RN . Pam will ensure your case is moving along properly, communicate with you throughout the course of care, help you anticipate a return to work date, and assist in returning your employee to work.

Referring into the Back to Work Center:

At the Back to Work Center, both therapists and specialty physicians work under the same roof and collaborate on all treatment considerations. This comprehensive approach leads workers’ compensation nurse case mangers and claims adjusters from around the state of Indiana to choose the Back to Work Center. To refer an injured worker to the Back to Work Center:
