Diabetes Education Treatment Information
Our educators will partner with you, your family and your doctor to help you manage your diabetes. We recognize each of our patients is different and require unique needs. Together, we will develop an individualized plan to help you learn skills on how best to control your form of diabetes based on your specific lifestyle, including:
- Healthy food choices and meal planning
- Value of physical activity
- How and when to monitor your blood sugar
- Diabetes medicines and how they work
- Coping with your feelings about having diabetes
- Preventing complications and overcoming barriers of care
We understand what a life-changing condition any form of diabetes can be. If you believe you may be at risk or have a family history of diabetes, we encourage you to get tested so you can begin intervention, education and treatment as soon as possible. Diabetes is an individual condition, but you don’t need to manage it alone.
