Home Care

At Indiana University Health Frankfort Hospital Home Care, our goal is to help you stay in your home by providing the care you need. We help you achieve the best quality of life possible, and we understand that one solution does not fit everyone’s needs. We listen to our patients and develop individualized care plans based on each person’s conditions and preferences.

Our services make it possible to receive hospital-based, professional care in your own home, helping you heal even after you leave the hospital. Home care is also available to patients who have not been admitted to the hospital.

Our services are available Monday – Friday, 8 am – 4:30 pm. A registered nurse can be reached 24 hours, seven days a week for emergencies and after hour inquiries. Our service area includes Clinton, Benton, Fountain, Montgomery, Tippecanoe and Warren counties.

IU Health Home Care services are delivered by highly trained medical professionals including nurses, physical therapists, occupational therapists, speech and language pathologists, medical social workers, dietitians, pharmacists and respiratory therapists. Our home care team works closely with each other as well as many area physicians to achieve goals set by the patient, caregivers and staff. With a combined total of over 25 years experience in home care, we work as a team to provide personalized care for each patient.

Our home care clinical services include comprehensive management of diabetes, chronic obstructive pulmonary disease (COPD) and other chronic conditions. We also offer home telemonitoring, wound care, stroke rehabilitation, cancer care, medicine management, post-operative management and patient and family education, and other services. When needed, we provide equipment, such as sleep apnea aids.
