Help us help you. Here are some tips to help your visit go smoothly:

  1. Use your legal name.
  2. Bring your insurance card to each visit.
  3. Inform us of a change in address, telephone number and/or insurance.
  4. Bring all of your medications.
  5. Verify all providers and services covered by your insurance plan.
  6. Call 24-hours in advance for prescription refills.
  7. Bring a list of your questions.
  8. Inform the physician if you need refills before leaving the office.
  9. If you have had tests and have not heard from us after 10-14 days, please call the office.
  10. If scheduling has not called you to set up your test or surgery as planned, please call your physician’s office.
