Registration and Pre-Registration

Why Pre-Register?

• Expedite the registration process in order to use time efficiently. We know your time is valuable.
• You can call 219.326.2407 or 800.235.6204 Monday through Friday from 8 am to 4:30 pm. Please leave a detailed message and a phone number where you can be reached.

What do I need to bring with me?

• Please bring your insurance cards, photo ID and a copy of your order if you have it.

Where do I go?

• You will go to the main registration desk on the first floor, unless instructed otherwise. They will activate your account, copy your insurance cards and photo ID, obtain your signature for consent to treatment and direct you to your point of service.

Special Services

• If you will need assistance (translator) please let us know before your date of service. Wheelchairs are always available at the main entrance.
