Healthcare Scholarship Golf Tournament Signup

Instructions:

  1. Please select your participation level, including your full name, diet preference, and t-shirt size. Click the "Signup" button. You will be taken to a PayPal shopping cart.
  2. If you wish to add purchases to your cart, click the "Continue Shopping" button.
  3. Click the "Add To Cart" button for any additional purchases you wish to add to your tournament entry. Repeat steps 2 & 3 until your PayPal cart is complete.
  4. Make sure the quantities for each option are correct in your PayPal cart, then complete payment on your transaction.
  • Gold Sponsor $1,000 (Registration, dinner and player favors for four golfers, tee sign and program recognition)
  • Player Sponsor or Individual Golfer $200 (Registration, dinner and player favors for one golfer.)
  • Tee Sign Sponsor $100 (Professionally prepared tee sign to promote your business. Please email your logo and/or 1 – 2 print lines by May 25.)
  • Mulligan Package $75 (Ten mulligans—the maximum allowed per team (can purchase single mulligans at event for $10.)
