VNA Hospice Care
Hospice Care is a philosophy of compassionate care for individuals with terminal or life-threatening illnesses. Hospice care is intended to provide comfort and relief from pain - physically, emotionally, and spiritually for both the patient and their family. It is based on the belief that these people should be able to live their lives to the fullest extent possible.
Hospice Care is a home-based service; however, sometimes home hospice care is not possible. As a result, the VNA offers Hospice care to local extended-care facilities and the Continuing Care Center of Indiana University Health La Porte Hospital.
Hospice care is coordinated through the patient’s primary physician and the hospice physician. Services include:
- Skilled nursing visits
- Home health aides
- Medical social services and counseling
- Symptom and pain management
- Spiritual/bereavement counseling
- Nutritional services
- Palliative pharmaceuticals
Hospice care also offers Volunteer Services to patients and families to assist with daily needs.
Hospice Care is covered by the Hospice Medicare Benefit and some private insurance. There are additional resources available for qualifying patients who are uninsured. For more information contact 219.871.8100 or 800.854.6008.
