Health Information Services Mission, Vision and Goals Statement

Mission: Manage health information from birth to death for individual patient records and aggregate data on patient populations in accordance with Indiana State and US Federal law guidelines. Our managment of health information is based upon clinical practices and informatics perspectives. Health information supports diagnoses and reimbursement validations.

Vision: Produce immediate access to comprehensive patient health information for care providers and other users of medical records.

Goal: Present accurate, reliable, and timely health information to assist healthcare providers with administering informative and quality treatment decisions regarding patient's health profile.

$1.00 per page for 1-10 pages
$0.50 per page for pages 11-50
$0.25 per page for pages 51 & higher
$10.00 for expedited requests within 2 days
$10.00 per Radiology CD
$10.00 per X-ray film
