Chronic Heart Failure Patients Now Eligible for Cardiac Rehabilitation

—The Indiana University Health System along with Cardiac Rehabilitation leaders are excited about the newly approved diagnosis of Heart Failure in Cardiac Rehabilitation programs. Thank you CMS! Currently, less than 1% of patients with a qualifying Heart Failure diagnosis are referred to Cardiac Rehabilitation programs at IU Health and we need your help. Please review the eligibility criteria below and help us achieve our system goal of 50% by referring your patients today.

Eligibility Criteria (must meet all components):

  • LVEF ≤ 35%
  • NYHA Classification of Class II to IV despite optimal HF therapy for at least 6 weeks
  • Patient must have “Stable Chronic Heart Failure”
    • Defined as:
      • Not having recent (≤ 6 weeks) major CV hospitalizations or procedures
      • Or, not having planned (≤ 6 months) major CV hospitalizations or procedures

New York Heart Association (NYHA) Functional Classification
**Eligible classes are those that are shaded and in bold**

Class Patient Symptoms
Class I (Mild) No limitation of physical activity. Ordinary physical activity does not cause undue fatigue, palpitation, or dyspnea (shortness of breath).
Class II (Mild) Slight limitation of physical activity. Comfortable at rest, but ordinary physical activity results in fatigue, palpitation, or dyspnea.
Class III (Moderate) Marked limitation of physical activity. Comfortable at rest, but less than ordinary activity causes fatigue, palpitation, or dyspnea.
Class IV (Severe) Unable to carry out any physical activity without discomfort. Symptoms of cardiac insufficiency at rest. If any physical activity is undertaken, discomfort is increased.
