Health insurance program offered by the State of Indiana for children from birth through age 18. It covers check-ups, well-child visits, doctor visits, hospital stays, and prescription drugs as well as vision and dental care. In general, there are no deductibles, co-payments, or premiums in Hoosier Healthwise for Children. There is an eligibility criterion based on family income.
How do I apply?
There is a one page application with two sides. You can apply at Riley Hospital for Children at Indiana University Health, on the first floor at the Children with Special Needs office or online at the Hoosier Healthwise website listed above. When you apply, make sure you have proof of your family's income, like tax forms or pay stubs (three of them). You will also need your child's Social Security number and in some cases, proof of his or her immigration status.

A supplemental insurance program that assists children from birth through 21 and who are financially and medically in need of health services as the result of chronic illness or disability. Must meet medical and financial criteria.
To apply, contact your local county Division of Family Resources, First Steps Agency, or Riley Hospital.
To complete the application, you’ll need these documents:
- Your child’s birth certificate
- Proof of residence (rent or property payment receipt, voter’s registration, driver’s license)
- Health insurance information
- Proof for all household members income (most recently filed federal income tax forms, three consecutive current paycheck stubs, all Government Award letters, private/public pension information)
A family with an income, before taxes, no greater than 250% of the federal poverty level may be eligible for the program. The financial level changes every year. For example, in 2012, a family of four could qualify with a gross income of up to $57,625.
For eligible applicants, services may include:
- Diagnostic evaluations
- Comprehensive well child and sick child care
- Specialty care and other services related to the eligible medical conditions
- Immunizations
- Prescription drugs
- Routine dental care

Program Description
A health insurance program for Indiana residents who experience problems obtaining or keeping health insurance due to a medical problem. ICHIA was created by the Indiana legislature to offer health care to residents of the state, through the participation of health insurance companies. The program administrator is Acordia Healthcare Inc.

Does not have or cannot have other insurance coverage
Has received one rejection notification from a health insurer
Please Note
If a person suffers from any specific medical condition on the ICHIA list, he or she does not need rejection notification.
Program Cost
As you enroll in ICHIA, you will be given four health plans to choose from. These plans differ in the amount of the premium, deductible, and coinsurance payment. These plans are described in detail in a brochure from ICHIA. You are also given a choice on the frequency of your payments; monthly, quarterly, semi-annually, or annually. Your rate is based on area, age and sex, and is subject to change. Medicaid is NOT accepted. Medicare is NOT accepted. Private insurance is NOT accepted.

Full-time medical staff of physicians, nurses and dietitians from the Pediatric Diabetes Team at Riley Hospital for Children at IU Health provides 24 hour care for campers.

Camp Until a Cure offers a full medical, nursing and dietary staff and a well trained counseling staff to provide diabetic care. Diabetes camp sessions are usually held several weeks each summer.
