Returning Home

If you have questions about continuing care after you leave the hospital, talk to your nurse or patient care coordinator. They will be happy to answer your questions. Your doctor will decide when you may be released from the hospital.

Guidelines for Hospital Discharge

• Talk with your doctor and nurse about your medicine.
• Read and sign your discharge instructions.
• Check to make sure you have all your personal belongings. If you need assistance packing, please ask your nurse.
• Retrieve any valuables left in the hospital safe.
• Know the time and location of when your ride will pick you up.
