Patients visiting IU Health Physical Therapy and Rehabilitation Services will receive compassionate, one-on-one care provided by licensed Physical Therapists and Physical Therapist Assistants. For more information or to schedule an appointment, please call 317.678.3301.

Our team approach includes physical therapy, occupational therapy and speech and language pathology. We will evaluate you or your loved one to determine your needs. That will help us create a program that is designed to help you achieve your goals.
This program may be right for you or a loved one if you are recovering and are facing problems such as:
- Difficulty walking
- Difficulty with balance
- Difficulty carrying out your daily activities due to
- weakness and/or shortness of breath
- difficulty adapting to your environment
- Needing help learning to use adaptive equipment (such as walkers and/or bathroom equipment)

- Wear comfortable, loose, clothing that you can move around in and allows access to injury or surgical site, if applicable.
- Arrive 30 minutes early to complete any outstanding paperwork.
In case your provider is not an IU Health provider, also please:
- Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail.
- If you’ve had any imaging procedures such as X-ray, CT or MRI, be sure to bring reports or films with you. You may also have them sent to us by your provider.
