From the moment you step into our office, we want to ensure you have a seamless and comfortable experience. During a typical appointment, our care team will do the following:

  • Discuss any concerns you may have
  • Review your medical and family history
  • Check your vitals
  • Provide the necessary medical support
  • Help you navigate any specialty care

During the initial appointment, you will meet with a physician who is board certified in pain management. The provider will do an extensive review of your health history, functional assessment and evaluate your current pain. Together you will develop a specialized treatment plan. You will have routine follow-up appointments as you work through your plan of care to assess and make changes accordingly.

Insurance

IU Health accepts most insurance plans. We recommend that you verify your benefits and eligibility with your insurance company before your visit.

Preparing for Your Appointment

Prior to your appointment, please bring the following:

  • Insurance and identification card(s)
  • Bring a copy of relevant medical records
  • A list of current medications, including over-the-counter medicines and supplements

Please arrive 15 minutes before your scheduled appointment to complete any outstanding paperwork.

All new patients will get a new patient packet mailed to them which includes information about the program and required paperwork they need to complete and bring to their initial appointment.

Parking is available onsite.
