IU Health Connected Care Lafayette is an outpatient primary care program designed to meet the unique needs of patients managing multiple health conditions. Connected Care is available by referral only.

Chronic conditions such as COPD (Chronic Obstructive Pulmonary Disease), heart disease, diabetes and dementia require a holistic approach to care that ensures each patient receives the best care possible with the most highly skilled providers.
Connected Care has a comprehensive care program team consisting of a:

- Advanced practice provider
- Registered nurse care manager
- Outpatient registered nurse
- Medical assistant
- Social worker
- Pharmacist
- Respiratory therapist
- Behavioral health consultant, as needed
- Dietitian, as needed
- Specialty consultant, as needed
