Preparing for Your Appointment

Prior to your appointment, please bring the following:

• Insurance and identification card(s)
• Bring a copy of relevant medical records
• A list of current medications, including over-the-counter medicines and supplements

Please arrive 15 minutes before your scheduled appointment to complete any outstanding paperwork.

We offer free on-site parking for patients. The Meridian Crossing location is south of IU Health North Hospital, across 116th Street.
