Access Medical Records with My IU Health
To have immediate access to an electronic version of your medical record, please create an account by selecting Create Account below. Once your account is set up, you can view your record at any time.
Create an AccountPrior to the end of your stay with us or upon discharge, you may request a copy of your medical records. IU Health saves medical records for a minimum of eight years. Medical records can be delivered via email, CD or paper form.
There is a three-step process for requesting copies of your medical records from IU Health.
- Download and print the Authorization to Release and Disclose Patient Information form. This form is standard across IU Health and can be used to request copies of your medical records at any of our facilities.
- Follow the instructions on the left side of the form to guide you in providing the type of information needed. Completing the form in its entirety will help ensure timely processing.
- Fax, email or mail the completed Authorization form to the IU Health facility from which you are requesting records. Facilities and contact information are listed below. If mailing, please include the facility name, address and “Attn: Health Information Management” on your mailing envelope.
Our goal is to process all requests within 5 business days upon receipt.
State and federal laws permit fees to be charged for copies of medical records.

How do I request my paper medical records from IU Health?

HIPAA has been modified for Health Oversight Activities, Law Enforcement Officials, Coroners and Medical Examiners, and in Judicial and Administrative Proceedings to submit with their request for protected health information potentially related to reproductive health care a valid Attestation form verifying the use or disclosure is not for prohibited purposes under 45 C.F.R. § 164.502(a)(5)(iii).
- Download and print the attestation form (PDF)
- Complete all blanks on the attestation form in its entirety, check the appropriate box and sign and date the written attestation form.
- Fax, email or mail the complete attestation form to the IU Health facility from which you are requesting records. If mailing, please include the facility name, address and “Attn: Health Information Management” on your mailing envelope.
