- Review your hospital stay and discharge instructions
- Discuss symptoms, questions or concerns since discharge
- Review your medical history and current medications
- Check vital signs
- Provide care and guidance based on your recovery needs
- Coordinate follow-up appointments, specialty care or additional services, as needed
- Ordering additional labs and radiology if needed
