What to Expect with IU Health Home Care Continuous Glucose Monitor Program
Step 1
Order Confirmation
You will receive a CGM order confirmation via email and/or phone within two business days of your provider referral.
Step 2
Client Agreement
Our IU Health CGM team will arrange for a client agreement signature.
Step 3
Equipment Sent to You
Your CGM equipment will be mailed to your door, along with a welcome packet and step-by-step instructions.
Step 4
Resupply Services
Ten days before you are eligible for a resupply, our IU Health CGM team will make contact with you to place an order.
Our team has a deep understanding of patient qualifications, billing and compliance regulations. We gather all necessary information for prompt prescription and insurance coverage processing.

- Ensure progress notes include the following qualifications to expedite processing:
- Frequency of blood glucose tests
- Frequency of insulin injections
- Date of last provider encounter

Frequently Asked Questions about CGMs
How do I know if I qualify for a CGM?
Typically, individuals with who is treat through insulin has a history of recurrent hypoglycemic events qualify for CGM. Talk with your provider to understand if CGM is the right fit for you.
What CGM products do you carry?
We carry the Dexcom G6, Dexcom G7, Freestyle Libre 2, and Freestyle Libre 3.
Do you provide insulin pumps?

Insurance and billing
What insurance plans does the IU Health CGM program accept?
We accept numerous commercial and government payer plans, including Medicare and Medicaid, making diabetes care management more accessible.
How much does a CGM device cost?
The cost of CGM therapy can vary greatly due to insurance coverage. Call your insurance company for details on what your plan covers. If you access CGM through a durable medical equipment (DME) benefit, you may have coinsurance, whereas accessing CGM supplies through pharmacy benefits means you'll be responsible for a co-pay. All of this is subject to meeting your deductible if you have one. If you're not insured, speak directly with your physician and the CGM manufacturer to see what options may be available to you.
