Step 1
Starting CGM Service
The CGM team will contact you within two business days of your provider referral.
Step 2
General Consent Form
Our IU Health CGM team will ensure that you have signed a general consent form.
Step 3
Insurance Authorization
We'll check your insurance to see what's covered and get approval if needed. You'll also find out ahead of time what costs to expect, so there are no surprises.
Step 4
Equipment Sent to You
Your CGM equipment will be mailed to your door, along with a welcome packet and step-by-step instructions.
Step 5
Resupply Services
Ten days before you are eligible for a resupply, our IU Health CGM team will make contact with you to place your resupply order.
Already using a continuous glucose monitor through IU Health Home Care? We offer digital reminders to help you stay on track with your CGM supplies so you never run out.
If you're on a 90-day resupply schedule, you'll get text and phone call reminders around day 80 after your last order. We'll have a few quick questions for you to confirm your next shipment. If something has changed — like your insurance or doctor's order — a team member will follow up with you directly.
You can also place resupply orders by calling 317.962.5310.
Important: Most insurance plans require that you see your doctor within six months of a CGM order — and every six months after that. Your doctor must also document how the CGM is helping you manage your diabetes and blood sugar levels. This helps avoid delays in getting your supplies.

Frequently Asked Questions about CGMs
How do I know if I qualify for a CGM?
To qualify for a CGM, you'll need to meet a few requirements:
- You have Type 1 or Type 2 diabetes.
- You've seen your doctor about your diabetes in the past six months, and continue to check in every six months to talk about how you're using your CGM and how it's helping manage your blood sugar.
- You take insulin or have a history of serious low blood sugar episodes (recurrent level 2 hypoglycemic events or one level 3 hypoglycemic event)
- Your doctor has talked to you about using a CGM and prescribed it to you to help manage your blood sugar.
What CGM products do you carry?
We carry the Dexcom G6, Dexcom G7, Freestyle Libre 2 Plus and Freestyle Libre 3 Plus.
Do you provide insulin pumps?
Not at this time.

Insurance and billing
What insurance plans does the IU Health CGM program accept?
We accept most insurance plans. Some may require you to get your CGM through a pharmacy — like an IU Health pharmacy — instead of through IU Health Home Care.
Our team will check your insurance and explain what's covered. You can also call your insurance company to ask if CGM supplies are covered through IU Health Home Care.
How much does a CGM device cost?
The cost of CGM therapy can vary greatly due to insurance coverage. Call your insurance company for details on what your plan covers. If you self refer or are referred to our CGM service by your provider, our team will review your insurance coverage and get approval if needed.
