If you qualify for an Annual Health Visit, you have the option to receive a health assessment at home or schedule an in-person visit with your primary care provider.

At IU Health, we offer patient-centered care that is accessible and convenient. Whether you prefer to come to one of our locations or have a medical professional visit your home, our program makes it easier for you to connect with your primary care provider and receive quality care.
If the visit is done at home, an IU Health nurse will call you to get your appointment scheduled. Once the visit is scheduled, an IU Health nurse will:
- Arrive at your home for your scheduled appointment
- Measure your vital signs
- Review your medications
- Answer any questions you may have
- Connect you to an IU Health primary care provider using a tablet

How do I schedule my appointment?
If you qualify for an Annual Health Visit, an IU Health team member will contact you to schedule your appointment. If you have any questions about your eligibility, please contact your primary care provider.
Do I have to have my Annual Health Visit at my home?
No. If you prefer to come in to the office, we can schedule you an appointment at your primary care provider’s office.
How will the Virtual Visit work?
Once the IU Health nurse has completed your health assessment, they will connect you to an IU Health provider through a tablet.
The provider will be able to answer any questions, advise on next steps and have face-to-face conversation with you through the screen.
Why do I need to schedule this visit annually?
Annual visits are beneficial to your overall health, allow IU Health to provide you the highest quality care, and makes sure IU Health is aligned with your Medicare plan each year.
