Refill Request Form Process

• You are eligible for a refill 28 days after your last shipment date.
• We recommend you place your order at least 3 business days before you need it.
• Refills are processed Monday through Friday 8:00 am to 4:30 pm EST. We do not process refills on weekends or holidays.
• If you need a same-day or next-day shipment, contact us before 2:00 pm EST:
  • Call us at 317.963.2400 or
  • Complete the refill form below and contact us by email at hcenteralrefills@iuhealth.org. Be sure to include the patient’s name and date of birth.
• You will receive a confirmation email after your order has been processed by Customer Service. The scheduled shipping date is based on when you are eligible for a refill.
• If your nutrition needs have changed and you are using more or fewer products than normal, please contact your physician. We need a new prescription faxed to 317.963.2091.
