Connected Care at IU Health Arnett is an outpatient primary care program designed to meet the unique needs of patients managing multiple health conditions.
In addition to a dedicated physician and nurse practitioner, the Connected Care team includes a case manager, patient care navigator, social worker, medical assistant and dietitian.
Support from ambulatory pharmacy and behavioral health round out the highly engaged care team. Connected Care is by referral only.
