IU Health Connected Care offers enhanced primary care services designed to meet the unique needs of adults on Medicare, Medicare Advantage and Medicare/Medicaid.
We are well-connected to you, your needs and your life. You'll have a compassionate care team of experts who will coordinate care with your specialists and deliver the personalized services that matter most to you.

Meet the Connected Care providers and learn more about our enhanced primary care services for adults on Medicare.
Our Connected Care team specializes in caring for adults on Medicare, whether you’re managing one or multiple conditions which may require more individualized care. Support staff are friendly, caring, experienced, and take time to get to know you and your unique needs. Longer appointment times are available to give you more time with you doctor if you need to discuss multiple chronic conditions, such as:
- COPD (chronic obstructive pulmonary disease)
- Heart disease
- Diabetes
- Dementia
Our care team includes:
- Physician
- Advanced Practice Provider
- RN Care Manager
- Patient Care Navigator
- Medical Assistant
- Social Worker
- Pharmacist
- Virtual Behavioral Health Services
- Dietician, as needed
- Specialty Consultant, as needed
Your care team will coordinate your care with pharmacy, behavioral health, social services and dietitian services as needed.
You will build a strong relationship with each member of your care team while staying connected with your specialty providers.
Our Services
Your doctor will work with you to create a customized care plan offering many enhanced primary care services to meet your needs, including:
- Longer appointments (30-60 minutes) to discuss multiple issues
- Same-day primary care appointments
- 24/7 phone access to a care provider
- Consistent and coordinated care team focused on getting to know patients and their unique needs
- Access to a pharmacist, behavioral health professional, dietician and social worker
- Virtual Visits (telehealth options)
- More customized, coordinated and convenient care based on the patient's individual needs
- Coordinated care with your specialty providers
- Improved management of Medicare patients with one or more conditions
- Reduced need for hospital and emergency care
- Connections with additional services (home health, caregiver and transportation)
We also provide caregiver support and education, as well as prescription management.
