IU Health Connected Care offers enhanced primary care services designed to meet the unique needs of adults on Medicare, Medicare Advantage and Medicare/Medicaid.
We are well-connected to you, your needs and your life. You'll have a compassionate care team of experts who will coordinate care with your specialists and deliver the personalized services that matter most to you.

Our Connected Care team specializes in caring for adults on Medicare, whether you’re managing one or multiple conditions which may require more individualized care. The care team is friendly, caring, experienced, and take time to get to know you and your unique needs. Longer appointment times (up to 60 minutes) are available to give you more time with you doctor. This gives you time to ask questions and discuss multiple chronic conditions, such as:
- Heart disease
- Diabetes
- COPD (chronic obstructive pulmonary disease)
- Dementia
- High blood pressure
- Depression
Our care team includes:
- Physician
- Advanced Practice Provider
- Pharmacist
- Care Manager (RN)
- Patient Care Navigator
- Social Worker
- Virtual Behavioral Health Services
- Medical Assistant
- Dietician, as needed
- Specialty Consultant, as needed
Your care team will coordinate your care with pharmacy, care management, virtual behavioral health, social work and dietitian services as needed.

What exactly is a geriatrician?
Dr. Wagle is a board-certified geriatrician. This means he is a primary care doctor who has received additional specialized training in diagnosing, treating and preventing age-related conditions and diseases affecting adult patients.
Who should see a geriatrician?
There is no right age to begin seeing a geriatric specialist, but a geriatric doctor can be helpful for patients managing multiple medications, conditions, or diseases associated with aging, such as dementia, incontinence or osteoporosis.
Who can go to Connected Care?
Any adult with Medicare, Medicare Advantage, or Medicare/Medicaid insurance coverage. If you or a loved one is managing one or more chronic conditions, Connected Care may be a good option for you.
Can I keep seeing my specialists?
Yes. Your Connected Care team will work directly with your heart doctor, diabetes doctor, and other specialists as needed. Together, they will help you receive the most coordinated care possible.
Is Connected Care in my network?
Yes. You’ll use the same insurance you have used with other IU Health providers.
Do you provide transportation?
Yes. We can coordinate transportation for all patients to and from appointments.
Will Connected Care become my new primary care provider?
Yes. Your Connected Care doctor and team will provide the enhanced primary care services you need, for as long as you need them.
