About Your Visit

We are happy you’re here. Let us walk you through the process of receiving support from our team of compassionate care givers. Inside our offices, you’ll find a team of therapists and peer recovery coaches to help you get started with our program.

Preparing for Your Visit

During the visit, we have a lot to cover. You’ll want to make the most of your visit with us. Before you arrive, it may be a good idea to:

  • Have your providers send your medical records to our office. They may send via fax or through the mail. You may also bring a copy of your medical records with you.
  • Bring your current list of medications. If you are unsure, bring your medications with you in a bag. We’ll go through them together.
  • Bring insurance card(s) and identification.
  • Arrive 15 minutes early to complete any outstanding paperwork.
  • If you have any questions about insurance coverage, please contact our office and we can assist you.

Parking

Parking is available directly outside of the Jay Medical Facility building. When you enter the building, the waiting room will be straight ahead.

If you have difficulty locating the building, don’t hesitate to call our offices for assistance.

What to Expect

When you arrive, you will be greeted by our compassionate staff. This will be your first stop to check in for your appointment, answer any questions and connect you to your provider.

Our staff will help to arrange your future sessions, reach out to you with appointment reminders and help you navigate the process of receiving your care.

If you’re coming in for your first appointment, you will complete a brief intake packet. If you’d like to do this ahead of time, you can find those in the Forms section on this page.

After you complete your initial paperwork you will meet with one of our clinical team members to complete your intake, treatment plan and create a strategy for your ongoing care.
