At our practice, we focus on providing care for patients who may need robust support and services. Consistent care is important for your health or the health of your loved one.   • Same-day primary care appointments
  • 24/7 phone access to a care provider

We also provide caregiver support and education, as well as prescription management.

To ensure that all patients receive adequate time with their provider, it is important that patients arrive on time for their scheduled appointment. If unforeseen circumstances arise and you may be late to your scheduled appointment, please call the office. You may be asked to reschedule your appointment if you arrive 15 minutes or later for your appointment.

From the moment you walk through our doors, we hope to provide smooth, kind and compassionate care. Beginning with registration, we will help guide you to your provider’s location, where you will be greeted by one of our clinical team members.

You will be led to a private exam room where you will meet with your provider and review your care needs. As you leave we will provide you with future appointments, instructions for testing and directions to your next destination, if needed.

IU Health accepts most insurance plans. We recommend that you verify your benefits and eligibility with your insurance company before your visit.

Preparing for Your Appointment

Prior to your appointment, please bring the following:

  • Insurance and identification card(s)
  • Bring a copy of relevant medical records
  • A list of current medications, including over-the-counter medicines and supplements

Please arrive 15 minutes before your scheduled appointment to complete any outstanding paperwork.
