Overview

Our doctors and healthcare experts focus on treating diseases and helping patients live healthier lives through preventive care and health education.

We also provide disease management services. These services include a lipid clinic and a clinic specifically for patients taking Coumadin. We also provide cardiology services for both adults and children on specific days of the week. We also have on-site lab services available.

For our patients’ convenience, we offer same-day appointments for sudden illness (as available). We also offer early appointments beginning at 7 am and late appointments until 6 pm on select days throughout the week (please see our office hours listed above). Free parking is available.

About Your Visit

From the moment you step into our office, we want to ensure you have a seamless and comfortable experience. When you arrive, you will be greeted by one of our registration staff, who will verify all of your information and let the clinical team know you have arrived for your appointment.

During a typical appointment, our care team will do the following so we can address what matters most to you:

• Discuss any concerns you may have
• Review your medical and family history
• Check your vitals
• Provide the necessary medical support
• Help you navigate any specialty care

Insurance

IU Health accepts most insurance plans. We recommend that you verify your benefits and eligibility with your insurance company before your visit.

Preparing for Your Appointment

Prior to your appointment, please bring the following:

• Insurance and identification card(s)
• Bring a copy of relevant medical records
• A list of current medications, including over-the-counter medicines and supplements

Please bring any completed paperwork that has been previously sent to you via mail or email in order to expedite your visit.

To ensure that all patients receive adequate time with their provider, it is important that patients arrive on time for their scheduled appointment. If unforeseen circumstances arise and you may be late to your scheduled appointment, please call the office. You may be asked to reschedule your appointment if you arrive 15 minutes or later for your appointment.
