  • Have your providers send any medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail. You may also bring copies of your medical records with you to your visit.
  • Bring insurance card (s) and identification
  • Bring a list of current medications, including over-the-counter medicines
  • If you’ve had any imaging procedures such as X-ray, CT or MRI outside of IU Health, be sure to bring reports and images on a disc
  • Arrive 15 minutes before your scheduled appointment time to complete any outstanding paperwork
