Acclimating to non-invasive ventilation (NIV): team supports and encourages patients every step of the way

For patients seeking breathing support from a non-invasive ventilator (NIV), their journey with the device can feel like a marathon, not a sprint, in their pursuit to breathe better.

“NIV devices reduce the work it takes to breathe,” says Matt Jaggers, respiratory therapist team lead. “It adapts to a patient’s breathing patterns which helps rest the muscles that are used to breathe. Patients often find the most challenges adjusting to their NIV mask and ventilator pressures during their initial weeks with the device. While some patients may experience immediate benefits, most typically require two to four weeks of consistent usage before noticing a significant difference.”

Each NIV patient has unique needs and preferences that are considered with their NIV care team at IU Health. Patients can wear a mouthpiece, face or nasal mask either throughout various times of day or as they sleep at night. The device is customizable to the patient’s needs and comfort levels. Pressure settings can be gradually adjusted to avoid overwhelming the patient. The NIV care team prioritizes the physical comfort and emotional needs of their patients to give them the best chance at long-term usage and success when acclimating to the device.

“Our respiratory therapists spend as much time necessary to ensure our patients feel comfortable with the equipment at the time of setup,” says Jaggers. “They discuss any fears or anxieties patients have about using the device and provide reassurance and information to alleviate those concerns as best as possible. We emphasize that prioritizing consistent usage of NIV is crucial, especially since it is primarily used to manage chronic conditions.”

Setting achievable milestones, such as increasing the duration of device use each day or week, can give the patient momentum and motivation to continue usage. Regular follow-ups from the NIV team help address any ongoing issues or discomfort from the patient. These check-ins are scheduled throughout the patient’s first 90 days and move to a quarterly basis after, but patients can reach out between each scheduled check in as needed.
