New remote patient monitoring pathway expands access in the Indy Health District

On a warm summer morning at a neighborhood farmers market, a Community Health Worker (CHW) greets residents with a simple offer: a quick blood pressure check and a conversation about heart health.

For some, that interaction becomes the first step into a new remote patient monitoring (RPM) pathway created through a partnership between IU Health and the Indy Health District.

Designed for adults with high blood pressure, the 16-week program combines community-based support with virtual care. Participants receive a blood pressure cuff, education on proper use and access to a smartphone platform that tracks readings and alerts the care team when follow-up may be needed.

The multidisciplinary care team includes CHWs, virtual registered nurses and advanced practice providers who support participants throughout the program.

The pathway builds on IU Health's established RPM model and extends it beyond traditional clinical settings to reach people who may not be regularly connected to care. CHWs can enroll eligible residents at community events and follow up during the first week to help them use the technology and better understand their blood pressure readings.

The program also helps connect participants to ongoing care. Enrollment is open to all eligible adults, not just IU Health patients. Care teams work with participants to establish or reconnect with a primary care provider based on their preferences.

Interest has been strong, with more than 25 residents enrolling during the program's first few weeks.

By meeting people where they are and combining trusted community partnerships with virtual care, IU Health is expanding access to blood pressure management and helping improve heart health across the community.
