Nurse spotlight: Courtney Jackson

Courtney Jackson is one of the discharge planners for University Hospital's organ transplant unit. She meets with patients as soon as possible after they're admitted to create a plan that will best help them succeed when they go home. "We want the patients to have a safe discharge and we want to prevent readmissions," said Courtney. "We work with the doctors, physical therapy, occupational therapy, a host of departments to either get patients the equipment they need when they go home, or to set them up with rehab placement if needed."
