Staff and therapists at IU Health Physical Therapy & Rehabilitation - Artistry are focused on making your experience as seamless as possible. We’re here to answer any questions and support you, from initial scheduling to follow-up appointments.

Insurance

IU Health accepts most insurance plans. We recommend you verify your benefits and eligibility before your visit.

Preparing for your first visit

To make the most out of your appointment, here are some helpful tips to help prepare you for your first visit.

  • Bring any post-operative instructions if you’ve recently had surgery.
  • Bring insurance card(s) and identification.
  • Bring a list of current medications, including over-the-counter medicines.
  • Wear comfortable, loose, clothing that you can move around in and allows access to injury or surgical site, if applicable.
  • Arrive 30 minutes early to complete any outstanding paperwork.

In case your provider is not an IU Health provider, also please:

  • Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail.
  • If you’ve had any imaging procedures such as X-ray, CT or MRI, be sure to bring reports or films with you. You may also have them sent to us by your provider.

Parking

Free parking is available in front of the building. Our parking lot is next to the dog park on Court St. between Cleveland St. and New Jersey St. Go down the set of stairs or ramp to the ground floor of our building. Once you enter the double doors, turn right for Suite B. Please indicate to the representative at the front desk whether you are there for Physical Therapy or Radiology.
