Our team approach allows us to work together to evaluate you or your loved one’s needs and create a program that is right for you. Our team of physical therapists, occupational therapists and speech and language pathologists will create a program for you.

This program may be right for you or a loved one if you are recovering and are facing problems such as:

  • Difficulty walking
  • Difficulty with balance
  • Difficulty carrying out your daily activities due to
    • weakness and/or shortness of breath
    • difficulty adapting to your environment
  • Needing help learning to use adaptive equipment (such as walkers and/or bathroom equipment)

We like to provide some helpful tips to look over before your first visit so that you can come prepared and make the most out of your appointment.

  • Bring any post-operative instructions if you’ve recently had surgery.
  • Bring insurance card(s) and identification.
  • Bring a list of current medications, including over-the-counter medicines.
  • Wear comfortable, loose, clothing that you can move around in and allows access to injury or surgical site, if applicable.
  • Arrive 15 minutes early to complete any outstanding paperwork.

In case your provider is not an IU Health provider, also please:

  • Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail.
  • If you’ve had any imaging procedures such as X-ray, CT or MRI, be sure to bring reports or films with you. You may also have them sent to us by your provider.
