Anticoagulation services are now available as part of our expanded Comprehensive Medication Review service.
Long-term anticoagulation therapy is used frequently in the management of certain vascular and clot forming conditions. This drug therapy is deemed high risk and therefore requires close monitoring to ensure safe and effective outcomes.
The IU Health Comprehensive Medication Review works to assess anticoagulation dosing, monitoring, and side effects. In addition, we educate patients to manage their treatment plan. Based on a referral from your physician, pharmacists will direct your anticoagulant care plan.
In addition to anticoagulation services, pharmaceutical services focused on medication education and affordability resources are available.

Preparing for Your Appointment
Prior to your appointment, please bring the following:
- Insurance and identification card(s)
- Bring a copy of relevant medical records
- A list of current medications, including over-the-counter medicines and supplements
