About Your Visit

When you come to our center, we want your experience to be as convenient and stress-free as possible. From the moment you walk in the door, we will welcome and guide you. We can help you with managing tests, scheduling appointments, and handling insurance issues. You should feel relaxed and comfortable while you are here.

We’re located at 21^st Street and Shadeland Avenue near the I-70 and I-465 interchange on the east side of Indianapolis.

Preparing for your visit

Here’s how to get ready for your appointment:

• Have your doctor send medical records to us before your visit or bring the information with you during your visit
• Bring insurance card(s) and identification
• Bring a list of current medications, including over-the-counter medicines
• If you’ve had any imaging procedures such as X-ray, CT or MRI, bring them with you
• Plan to arrive 15 minutes early
