The dedicated therapists and staff members will make sure your experience goes as smoothly as possible, from initial visit to follow-up appointments.
During your first appointment, you’ll meet with members of your physical therapy team, who will work closely with you to work on your specific mobility issues. Typically, physical therapy works over time, so you’ll be scheduled for a series of appointments.
Insurance
IU Health accepts most insurance plans. We recommend that you verify your benefits and eligibility before your visit.
Preparing for your first visit
To make the most out of your appointment, here are some helpful tips to help prepare you for your first visit.
- Bring any post-operative instructions if you’ve recently had surgery.
- Bring insurance card(s) and identification.
- Bring a list of current medications, including over-the-counter medicines.
- Wear comfortable, loose, clothing that you can move around in and allows access to injury or surgical site, if applicable.
- Arrive 30 minutes early to complete any outstanding paperwork.
Please provide 48-hours notification to reschedule or cancel an appointment.
In case your provider is not IU Health affiliated, please:
- Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail.
- If you’ve had any imaging procedures such as X-ray, CT or MRI, be sure to bring reports or films with you. You may also have them sent to us by your provider.
Parking
We are located on the northwest corner of 146th Street and Hazel Dell in Noblesville. You will enter off of Hazel Dell. Drive around to the front of the building and enter the double sliding glass doors which face 146th Street. Once you enter the building, we are straight back in Suite 110.
