How to Schedule an Appointment
A referral from your provider may be needed before your visit. You may call the IU Health central physical therapy and rehabilitation scheduling phone number at 317.962.9830 or contact us directly at 317.768.6045.
Please provide a notification within 48-hours to reschedule or cancel an appointment.
Our goal is to build a supportive relationship with you to promote your health and well-being. Because your challenges are unique to you, your treatment needs to fit into your lifestyle.
During your first appointment, we’ll talk about your challenges and discuss a therapy plan that works for you. This may include a combination of therapies, and a number of appointments. This allows us to ensure you are building up your stamina and learning to move safely to prevent future injury.
Subsequent visits include one-on-one training and therapy. We’ll evaluate you along the way and monitor your progress.
Insurance
IU Health accepts most insurance plans. We recommend that you verify your benefits and eligibility before your visit.
Preparing for your visit
To make your appointment as seamless as possible, here are some helpful tips to help prepare you for your first visit.
- Bring any post-operative instructions if you’ve recently had surgery.
- Bring insurance card(s) and identification.
- Bring a list of current medications, including over-the-counter medicines.
- Wear comfortable, loose, clothing that you can move around in and allows access to injury or surgical site, if applicable.
- Arrive 30 minutes early to complete any outstanding paperwork.
In case your provider is not an IU Health provider, also please:
- Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail.
- If you’ve had any imaging procedures such as X-ray, CT or MRI, be sure to bring reports or films with you. You may also have them sent to us by your provider.

Free parking is available on-site. We are located off of Whitestone Parkway in Zionsville.
