We want everything to go smoothly for you. Here are some helpful tips to help you get ready for your appointment with us.

  • Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail. You may also bring copies of your medical records with you to your visit.
  • Bring insurance card(s) and identification.
  • Bring a list of current medications, including over-the-counter medicines.
  • If you’ve had any imaging procedures such as X-ray, CT or MRI, outside of IU Health be sure to bring reports and images on a disc.
  • Arrive 15 minutes early to complete any outstanding paperwork.
