A referral from your provider may be needed before your visit. You may call the IU Health Physical Therapy & Rehabilitation Central Scheduling phone number at 317.962.9830 or contact us directly at 317.890.5534. Please provide a 48-hour notification to cancel or reschedule an appointment.

Our therapists and staff members are dedicated to making your experience at IU Health Physical Therapy & Rehabilitation in Methodist Medical Plaza East location as seamless as possible.
You’ll work with a team of therapists, who will ask you a series of questions to understand your condition and your treatment goals. We’ll review any previous medical records or radiology results, and evaluate your overall health and fitness.
Once we establish your needs, we’ll work together to design a treatment plan that’s as unique as you. You’ll be informed every step of the way, so you know what we expect of you, and what to expect from every appointment.
Insurance
IU Health accepts most insurance plans. We recommend that you verify your benefits and eligibility before your visit.
Preparing for your first visit
Before your appointment, we want you to know some helpful tips to help prepare. Here are some tips we have listed below:
- Bring any post-operative instructions if you’ve recently had surgery.
- Bring insurance card(s) and identification.
- Bring a list of current medications, including over-the-counter medicines.
- Wear comfortable, loose, clothing that you can move around in and allows access to injury or surgical site, if applicable.
- Arrive 30 minutes early to complete any outstanding paperwork.
In case your provider is not an IU Health provider, also please:
- Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail.
- If you’ve had any imaging procedures such as X-ray, CT or MRI, be sure to bring reports or films with you. You may also have them sent to us by your provider.
Parking
Free parking is available immediately outside of the building. When you come through the sliding doors, head to the right of the desk in the middle of the lobby. We are Suite 115, just to the tight of the elevators.
