Our locations are operating at a level to maintain social distancing and ensure that both team members and patients have adequate personal protective equipment. Masks will be provided for each patient and approved visitor.

We will continue to answer voice messages and provide other clinical support via phone to established patients during regular business hours. If you have any scheduling questions, please call the clinic directly for assistance.

Visitor Restrictions

To better protect our caregivers, team members and patients, IU Health is restricting visitors at all locations and service areas across. Children and guest are prohibited to attend with an adult patient.

Exceptions – One visitor allowed for:

  • A guardian with a minor patient
  • Adult patients with disability, who need assistance due to the specifics of their disability

We offer expert outpatient care for individuals who have had COVID-19. Especially if you had a long hospital stay, returning home and getting back to normal life may be difficult.

Our team approach includes physical therapy, occupational therapy and speech and language pathology. Our therapists will evaluate you or your loved one to determine your needs. We'll create a program to help you achieve your goals.

This program may be right for you or a loved one if you are recovering and are facing problems such as:

  • Difficulty walking
  • Difficulty with balance
  • Difficulty carrying out your daily activities due to
    • weakness and/or shortness of breath
    • difficulty adapting to your environment
  • Needing help learning to use adaptive equipment (such as walkers and/or bathroom equipment)
  • Breathing difficulties
  • Difficulty using your hands for activities such as writing or cutting food
  • Vision problems
  • Difficulties with thinking such as
    • memory
    • reasoning

  • Difficulty with swallowing

You’ll work with a team of therapists, who will ask you a series of questions to understand your condition and your treatment goals. We’ll review any previous medical records or radiology results, and evaluate your overall health and fitness.

Once we establish your needs, we’ll work together to design a treatment plan that’s as unique as you. You’ll be informed every step of the way, so you know what we expect of you, and what to expect from every appointment.

How to schedule an appointment

A referral from your provider may be needed before your visit. You may call the IU Health Physical Therapy & Rehabilitation Central Scheduling phone number at 317.962.9830 or contact us directly at 317.890.5534. Please provide a 48 hour notification to cancel or reschedule an appointment.

Insurance

IU Health accepts most insurance plans. We recommended that you verify your benefits and eligibility before your visit.

Preparing for your first visit

To make the most out of your appointment, here are some helpful tips to help prepare you for your first visit.

  • Bring any post-operative instructions if you’ve recently had surgery.
  • Bring insurance card(s) and identification.
  • Bring a list of current medications, including over-the-counter medicines.
  • Wear comfortable, loose, clothing that you can move around in and allows access to injury or surgical site, if applicable.
  • Arrive 30 minutes early to complete any outstanding paperwork.

In case your provider is not an IU Health provider, also please:

  • Have your provider send medical records to us before your scheduled visit or bring the information with you during your visit. These may be sent via fax or through the mail.
  • If you’ve had any imaging procedures such as X-ray, CT or MRI, be sure to bring reports or films with you. You may also have them sent to us by your provider.

Parking

    Free parking is available immediately outside of the building. When you come through the sliding doors, head to the right of the desk in the middle of the lobby. We are Suite 115, just to the tight of the elevators.
