We can help you find a path to care to begin your steps toward recovery and connect you to the services you need.

The IU Health Community Health Family Vitality Initiative is a free program in the South Central region of Indiana that is focused on helping pregnant moms have healthy babies.

Indiana has the highest rate of infant deaths in the Midwest. Over 3,000 infants have lost their lives before their first birthday in the past two years.

We can help reduce that risk for your baby by helping you navigate to needed care during pregnancy and once the baby is born.

About Family Vitality Initiative

The IU Health Community Health Family Vitality Initiative is a free program in the South Central region of Indiana that is focused on helping pregnant moms have healthy babies.

If we can help you early in your pregnancy, we have a better opportunity to help you and others in your home get the treatment you need. This is the start of a path toward a healthier life for you and your baby.

Who Can Benefit from Our Program?

If you are expecting a baby and experiencing issues with substance misuse, you can benefit from our program. While our goal is to link mom to services, we will also assist others living in the home who may be experiencing substance issues. We’ll help them find treatment options as well.

If you and other caregivers to the baby are participating in treatment, this can reduce risks of infant death and promote a better opportunity for your continued success in recovery.

We will also assist moms who have not yet been connected to prenatal care begin that journey. Receiving recommended care during pregnancy can help your baby with a healthier start to life. We are here and able to assist in getting your initial visit scheduled if needed.

How Much Does the Program Cost?

There is no cost for families enrolled with Family Vitality Initiative.

We provide navigation services to clients free of charge to help connect them to needed medical treatment during pregnancy, substance treatment, medical treatment for the baby after its birth and any social services assistance they may need to promote the health of their baby and family.

What are the Advantages of Enrolling in Family Vitality Initiative?

In addition to navigation with healthcare, we work to help connect you to other community partners that can help assist you with additional needs you may have.

Having Your First Baby and Under 28 Weeks Pregnant?

If so, you may qualify for the IU Health Community Health Nurse-Family Partnership program. In this program, you can have a personal nurse alongside you during pregnancy and up to your child’s second year of life. This program is also free of charge.

Struggling or Unsure How to Apply for Medicaid Services or for Food Stamps?

We have a free community partner to assist with that. We want to help you navigate issues that could improve your ability to promote a healthy environment for your child.
