This page is designed as a resource for educational institutions that either have an educational affiliation with IU Health or desire to enter into an affiliation agreement for student placement. 

The page also serves as a resource for students from affiliated educational institutions that will be completing an educational rotation at IU Health.

IU Health requires all students/faculty to provide certain information prior to starting an educational rotation. The required information is as follows:

  1. Complete a background check that includes a national criminal check, a check of sex offender registries and a check of the federal exclusions lists. Access a full list of criminal offenses which make a student/faculty member ineligible to participate in an educational experience. 
  2. Have the following vaccinations: MMR: measles, mumps, rubella; Tdap: tetanus, diphtheria, pertussis; annual influenza; Hepatitis B (or declination form); and varicella; or show appropriate antibody titers.
  3. Has had a TB test within the last 12 months.
  4. Have successfully passed a drug screen.
  5. Has a current American Heart Association CPR/BCLS card if required by the clinical area where the experience will be conducted.
  6. Have completed either quantitative or qualitative fit testing for a N95 respirator (only applicable if student will be working in a negative pressure room). 
  7. Student/Faculty must possess his/her own health insurance.

Student must provide verification of the above items by completing the Clinical Student Validation Form and submitting required documentation when requested by IU Health.

If the matrixes above do not indicate an agreement is already in place for a particular school or program, complete the request form below.

Office representatives will work with Academic Affairs leadership and the clinical area to determine if requests can be granted. If an agreement can be granted, the contact person from that office will coordinate the processing/negotiations of the educational affiliation agreement with the educational institution.  This process will utilize the current IU Health Educational Affiliation Agreement template that has been approved by IU Health Legal Services.
